Module migration 

Port of internal/config/migration.go: structural upgrade of a config file to the latest schema, in memory (PlanMigration) or committed to disk (MigrationPlan::write).

plan_migration

Reads the config at path and computes its upgrade to the latest format without writing anything. Returns ConfigError::AlreadyMigrated if the file is already at the latest version.
